Faith Formation, Gr. 1-4

Please complete an individual form for each child.  Faith Formation Registration Suggested Donation (Materials Fee): $15/student, $35 max per family with students in preschool – Grade 4.  You can either pay online or mail your check to Nativity at 3312 Silver Lake Rd., Minneapolis, MN 55418.  Please designate the type of payment you are using.   28. Parents and other adults lead class experience with their child. Parents are encouraged to share the class experience with their child. Workshop Guides repeat their workshop for different grade groups each week of the session. Workshop may be drama, computer, art, cooking, Bible study. Grade-level Faith Coaches stay with the same grade and experience different workshops each week for the 4-8 weeks of the session. Check your family calendar and select one or two sessions below when you could help:
