Faith Formation Classes Preschool Registration

Please complete the following Registration Form (one per child) for Preschool/Kindergarten Faith Formation classes - ages 3 through Kindergarten (child needs to be 3 no later than August 31, 2010 and potty trained.)

You may pay the suggested donation for materials of $15 per child/$35 max per family on line or you can mail a check made out to Nativity Lutheran Church with "Faith Formation" on the memo line to:

When choosing a class time, keep in mind:
9:00 class sizes are smaller.
10:30 classes are larger requiring more adults.
Sat. 5:30 class is a mixed-age class of students age 3 - Kindergarten.
